Why Do Adult Briefs Leak? Fit, Size and Change Timing

· 3 min read

Poor fit, including the wrong size or a brief that is not put on correctly, is a common reason briefs leak1. A product with too little absorbency for the leakage can also leak, and you can try another product if yours does not keep you dry enough2. This article covers fit, putting a brief on, changing and what to ask your provider. It is general information.

Why does a brief leak?

  • Wrong size. NAFC says most leaks and skin irritation are caused by improper fit1. Use the size chart on your product's package. Sizing can be harder with large thighs, amputations or hernias1. MedlinePlus advises a size that fits snugly2.
  • Not put on correctly. NAFC describes a good fit as the brief lying flat across the abdomen with the wetness indicator at midline, the top fasteners angled down to tighten the waist, and the bottom tabs angled slightly up to tighten the leg gathers. There should be no gaps at the legs1. If the brief does not cover both cheeks, expect wet clothing or a wet bed1.
  • Not enough absorbency. MedlinePlus says the right product depends on how much leakage you have and when it happens, and that you can try another product if yours does not keep you dry enough2. If leaks happen overnight or in large amounts, ask your provider about a different product or absorbency level.
  • Change timing. NAFC cautions that the wetness indicator is a prompt to check the brief, not a precise gauge, and that sweat or moisture can also change its color1. Clean and dry the skin right away after urine or a bowel movement3. Ask your provider how often to change a brief.
  • Lying on your side at night. Continence Product Advisor reports that some wearers find all-in-one pads perform poorly when lying on their side in bed, because they have little absorbent material at the sides. It suggests absorbent bed pads and a waterproof mattress protector for night leaks6.

When to contact a provider

Contact your provider if you are not staying dry, or if you are not sure how to use your product2. Contact them for a skin rash or sores, or signs of infection such as a burning sensation when you urinate, fever or chills2. Also contact them for cloudy or bloody urine, pain when you urinate or trouble starting your urine flow5. Ask a provider about the cause of leakage and possible treatment; products can be used alongside treatment. MedlinePlus says a provider might ask you to drink less fluid to cut down on leakage2; do not cut back on fluids unless your provider tells you to. Keeping a journal of when leaks happen can help2. Mayo Clinic says bed-wetting that starts in adulthood is not typical and may be a sign of a health condition4.

Coverage and questions

Coverage depends on the payer and program. Our guides explain New York Medicaid and incontinence supplies and Medicare and adult diapers. Questions about your benefits? Ask us at our Mount Kisco store.

Protect the skin

Constant contact with urine or stool can break down skin3. Clean with mild, dilute soap and water, rinse well and pat dry, or use a soap-free cleanser as the product instructs3. Barrier creams or skin sealants can help; your provider can recommend one, and the skin must still be cleaned each time3. Contact your provider if severe skin irritation develops3.

When to get emergency help

MedlinePlus says to call 911 or go to an emergency room if you suddenly lose control over urine and also have difficulty talking or walking, sudden weakness, numbness or tingling in an arm or leg, loss of vision, loss of consciousness or confusion, or loss of bowel control5.
