The provider form collects practice and provider details, NPI, staff name, and staff contact information. Do not submit patient records, insurance information, clinical information, or order details through these forms. Contact us for an approved submission method.

We use information submitted through our website forms to respond to you and arrange next steps. If you later provide patient, insurance, clinical, or order information through an approved submission method, we may use that information to verify eligibility, obtain required documentation, submit claims or prior authorizations, deliver equipment, manage resupply, and contact you about an order.

As a durable medical equipment supplier we handle protected health information in accordance with federal and New York health privacy law. We disclose PHI only as needed for treatment, payment, and healthcare operations, or where required by law.

If you provide patient, insurance, clinical, or order information through an approved submission method, we may share it with the applicable insurance plan, prescribing clinicians, and vendors involved in manufacturing or shipping equipment, as needed to provide services or as permitted by law.
