Content on this site is general information about insurance coverage for medical equipment. It is not medical advice and does not replace guidance from your clinician. Always follow your prescriber's instructions.
Coverage estimates
Benefit verifications are based on information provided by your insurance plan at the time of the check. Final coverage and patient responsibility are determined by your plan when the claim is adjudicated.
Prescriptions and eligibility
Most equipment requires a valid prescription and documented medical necessity. We may be unable to fulfill an order if required documentation cannot be obtained or if your plan denies coverage.
Billing
We bill your insurance plan directly where we are contracted. You are responsible for deductibles, coinsurance, and copays as determined by your plan, and for non-covered items you have agreed to purchase.
Returns and equipment condition
For health and safety reasons many supplies cannot be returned once shipped. Defective equipment is repaired or replaced in accordance with manufacturer warranty and payer rules.
Limitation of liability
To the extent permitted by law, Medically Modern is not liable for indirect or consequential damages arising from use of this website.
